SHORT NOTICE Cancellations & Missed Appointments (Non-NDIS PATIENTS)
Blackwood Podiatry requires at least 24 hours notice to cancel or reschedule an appointment.
If less than 24 hours notice is given, a $33 late notice fee or $66 no-show fee may be charged, payable by electronic funds transfer or at your next appointment.
Please note cancellation fees are not covered by third-party organisations or private health insurers and must be paid directly by the patient.
NDIS NOTICE Cancellation POLICY
Short notice cancellations for NDIS participants are billed per the NDIS Price Guide.
A minimum of 48 hours notice is required to cancel or reschedule NDIS appointments at Blackwood Podiatry; otherwise a cancellation fee of up to 100% of the appointment cost may apply.

Privacy Statement
As a patient of our clinic, we require you to provide us with your personal details and full medical history, so that we may properly assess, diagnose, treat and be proactive in your health care needs.
We comply with the Privacy Act 1988 (Cth), the Health Records (Privacy and Access) Act 1997 and other legislation that regulates how personal information is handled, including all your health details with any sensitive information.

Blackwood Podiatry has a privacy policy that outlines how we collect, use, disclose and store your information. Our policy also contains details about how you can ask what information we have, request access to, advise corrections and make changes to your personal information. We will only collect information to:
• Provide you with podiatry services;
• Enable the administration and running of our practice;
• Disclose to other podiatrists in the practice for patient care and teaching;
• Disclose to others involved in your healthcare, including treating doctors and specialists outside this practice. This may occur through:
• referral to other doctors/health practitioners,
• referral for medical tests,
• reports or results sent between treating clinicians,
• disclosure to health practitioners in the event of a medical emergency;
• Send reminder letters regarding your health care and management;
• Complete any insurance or compensation claim;
• Comply with any legislative or regulatory requirements.

Informed Consent
As part of our duty of care, your podiatrist will provide a clear explanation of your diagnosis and the proposed treatment, including any possible complications or risks associated with the treatment.

From time to time we run competitions and promotions on our social media profiles. It’s our way of giving something back to our patients. Below are general terms and conditions for these competitions:
By entering any competition, entrants consent to the use of their personal photos by the Promoter for the purposes of promotion and in commercial use.
This promotion is in no way sponsored, endorsed or administered by, or associated with, Facebook or Instagram.
You must not inaccurately tag content - you may not tag or enter content that you do not expressly own.
Blackwood Podiatry accepts no responsibility for any entries that are incomplete, illegible, corrupted or fail to reach us by the relevant closing date for any reason. Proof of sending is not proof of receipt. Entries via third parties are invalid.
Competitions are open to residents of Australia, aged 18 years or over. Employees of Blackwood Podiatry or its parent company, as well as the immediate family (spouse, parents, siblings, children) and household members of each such employee and/or authors, are not eligible to participate in the Promotion.
The prize for the winner is as detailed and outlined in any promotion.
The Promoter’s decision is final. No correspondence will be entered into.
The prize is non-transferrable and no cash alternative will be offered.
The winner must claim their prize within 30 days of Blackwood Podiatry sending notification. If the prize is unclaimed after this time, it will lapse and the Promoter reserves the right to offer the unclaimed prize to a substitute winner selected in accordance with these rules.
