The information below helps prepare you for your first podiatry appointment with us.

LOCATION & PARKING

Blackwood Podiatry is located in Shop 10 Shepherd Court, 365 Shepherds Hill Road, Blackwood. You will find our clinic entrance on Waite Street next door to the Vietnamese Tasty Meat Rolls shop.

There is parking available within the Shepherd Court group of shops (off Shepherds Hill Road). Alternatively, you can park across the road in the Drakes back car park (off Waite Street).

ACCESSIBLE PARKING

There is one accessible parking space in Shepherd Court (in front of Anytime Fitness).

We suggest that patients who have difficulty parking in the main car park arrange to be dropped off and picked up again in the back laneway next to Blackwood Podiatry (off Waite Street). It’s an easy few metres from the laneway to our clinic door.

People who use wheelchairs can access Blackwood Podiatry directly through our front clinic door.

NEW PATIENT REGISTrATION FORM

You will need to complete a new patient registration form before your first appointment. This information helps us to provide you with the best quality care.

You can print out the form below, fill it in and bring it with you to the appointment. Alternatively, arrive 10 minutes early and we will provide you with a copy to complete.

• Blackwood Podiatry New Patient Form (Adult 16+ years of age)

• Blackwood Podiatry New Patient Form (Child <16 years of age)

BEFORE YOUR APPOINTMENT

• Please remove any nail polish, acrylic nails, or shellac if you intend for the podiatrist to assess or treat your toenail(s). Unfortunately, our nail care instruments can not be used if your nails are coloured.

• Wear comfortable clothing suitable for lower limb assessment, such as shorts or pants that can easily be rolled up.

• We recommend making a list of any symptoms or questions you’d like to cover with your podiatrist.

• If you have special needs or questions, please don’t hesitate to contact our clinic to discuss with one of our friendly receptionists.

PLEASE BRING WITH YOU…

• A range of shoes that you wear regularly - including work shoes, school shoes, sports shoes, as well as footwear that you wear around the house.

• Relevant medical records, test results, x-rays, MRI scans, or ultrasounds.

• A list of your current medications (if you have not filled in the new patient form before arrival).

• Any previous orthotics or insoles.

• Your private health fund card (if applicable).

• Any referral paperwork (if applicable).

WHAT TO EXPECT AT YOUR FIRST APPOINTMENT

During your first appointment, your podiatrist will complete a comprehensive foot examination.

Together, you will develop a customised treatment plan tailor-made for your foot health and lifestyle.

If required, treatment usually commences during the first consultation (if time allows).

PAYMENT OPTIONS

Payment is required on the day of consultation. We accept the following payment options:

• Cash

• Debit card

• Credit cards: Visa, Mastercard, and American Express

We offer a HICAPS facility, which allows you to claim your treatment costs at the time of your consultation if you are privately insured.

Medicare rebates are available to those with a valid Chronic Disease Management Plan referral.
